Stimulate the patient and administer naloxone as prescribed. Naloxone is an opioid antagonist that reverses respiratory depression. The typical initial dose is 0.4 mg to 2 mg IV, repeated every 2 to 3 minutes as needed, with a lower dose for postoperative patients to avoid a sudden return of severe pain.
Maintain a patent airway and provide ventilatory support with a bag-valve-mask delivering 100% oxygen if the patient is not breathing adequately. Continuous monitoring of oxygen saturation and end-tidal CO2 (capnography) is essential, as capnography provides the earliest indication of respiratory depression.
Do not rely solely on pulse oximetry; a drop in SpO2 is a late sign of hypoventilation. Sedation always precedes respiratory depression, so a sedated patient requires immediate assessment of their respiratory status.
