Sedation is the earliest sign of opioid toxicity and always precedes respiratory depression. A respiratory rate below 10-12 breaths/min is a critical late sign.
Only the patient should press the PCA button. A sleeping patient with pain will not self-administer; proxy activation by family or staff bypasses the body's protective sedation mechanism and can cause fatal overdose.
