An irregular breathing pattern with apneic pauses signals a compromised airway and failing respiratory drive, which can lead to irreversible brain damage from hypoxia within minutes.
This breathing pattern may indicate Cushing's triad, a late sign of increased intracranial pressure (ICP) affecting the brainstem. The immediate nursing action is to ensure airway patency and adequate ventilation, often starting with a jaw-thrust maneuver and preparing for assisted ventilation.
Do not be distracted by hypertension or fever. While important, they are secondary to the immediate threat of respiratory arrest. A systolic BP of 160 may be a compensatory mechanism to maintain cerebral perfusion and should not be the first priority treated.
