학습 자료 둘러보기 →Decerebrate posturing is a late, ominous sign of transtentorial herniation and severe brainstem compression. It indicates damage below the red nucleus and requires immediate intervention to prevent respiratory and cardiac arrest.
Prioritize this finding over other signs of increased ICP like pupillary changes or Cushing's triad. A progression from decorticate to decerebrate posturing or a unilateral to bilateral presentation signals a neurological emergency.
Do not confuse decerebrate posturing with decorticate posturing (flexion of arms, extension of legs), which indicates a lesion higher in the brain. Decerebrate posturing carries a significantly worse prognosis and demands an escalated response.
