Implement bleeding precautions immediately: use a soft-bristled toothbrush, avoid rectal temperatures and intramuscular injections, apply prolonged pressure after venipuncture, and maintain a clutter-free environment to prevent falls.
Monitor closely for signs of occult bleeding such as a drop in hemoglobin, hypotension, or new onset of severe headache. Avoid activities that increase intracranial pressure, including vigorous coughing or straining.
