A hemoglobin of 6.8 g/dL with symptoms of shortness of breath and dizziness indicates profound symptomatic anemia and tissue hypoxia, requiring immediate packed red blood cell transfusion.
While neutropenic precautions (for WBC 2,100/mm³) and bleeding precautions (for platelets 45,000/mm³) are critical, they address potential future complications. The active, life-threatening hypoxia from severe anemia takes precedence.
Administering oxygen alone is a supportive measure that does not fix the underlying lack of hemoglobin. Monitor closely for transfusion reactions (fever, chills, urticaria, dyspnea) during the first 15 minutes of the transfusion.
