학습 자료 둘러보기 →Kawasaki disease (KD) is a leading cause of acquired heart disease in children. The acute phase (days 1-14) is defined by high fever unresponsive to antipyretics and at least four of five principal clinical features. The mnemonic "WARM CREAM" helps recall these criteria: Conjunctivitis (bilateral, non-exudative), Rash (polymorphous), Adenopathy (cervical, often unilateral >1.5 cm), Mucosal changes (strawberry tongue, cracked lips, pharyngeal erythema), and Extremity changes (erythema and edema of palms/soles).
Nurses must differentiate acute from subacute findings. Acute indicators include the fever and the "CREAM" signs. Subacute findings (weeks 2-4) include periungual desquamation of fingers and toes, thrombocytosis, and the development of coronary artery aneurysms. Prompt recognition and administration of IVIG and high-dose aspirin within the first 10 days of fever significantly reduce the risk of coronary complications.
Key Nursing Actions: Monitor cardiac status with echocardiogram, manage fever, provide comfort for mucositis and skin irritation, and educate families on the importance of follow-up echocardiograms and potential for irritability lasting weeks.
