Immediate nursing actions include obtaining blood cultures (peripheral and from any central line) and a urinalysis with culture before administering broad-spectrum antibiotics. Notify the transplant team immediately. A temperature above 100.4°F (38°C) is a critical threshold.
Do not delay antibiotic administration for culture collection. In an immunocompromised host, a localized infection can disseminate within hours. Monitor closely for signs of septic shock, including hypotension and altered mental status.
