This is to detect a biphasic reaction, a recurrence of symptoms without re-exposure that can occur in up to 20% of cases, typically within 1 to 72 hours.
Epinephrine’s therapeutic effects are temporary due to its short half-life. While it provides rapid vasoconstriction and bronchodilation, the underlying inflammatory cascade can reactivate. A second dose is not given prophylactically but only if symptoms return.
Do not assume the reaction is over because of initial improvement. The airway, breathing, and circulation can deteriorate rapidly and without warning. Ensure emergency equipment remains at the bedside.
