Stridor is a high-pitched inspiratory sound indicating critical narrowing of the upper airway. When accompanied by difficulty speaking or dysphonia, it signals that the airway lumen is severely reduced and total obstruction may be imminent.
While urticaria, angioedema of the face, and gastrointestinal symptoms confirm a systemic reaction, they do not constitute immediate airway compromise unless stridor or respiratory distress is present.
Stridor in anaphylaxis is a trigger for immediate intervention. Administer intramuscular epinephrine without delay and prepare for airway stabilization. Do not wait for further deterioration, as progression to complete obstruction can be rapid.
