The priority assessment finding is hypotension (e.g., systolic BP <90 mmHg) with a compensatory tachycardia (weak, rapid pulse), indicating hemodynamic instability and organ hypoperfusion.
While cutaneous (hives), gastrointestinal (nausea), or mucosal (facial swelling) signs confirm an allergic reaction, they do not define shock. Circulatory collapse is the most immediate life threat and requires rapid administration of intramuscular epinephrine into the anterolateral thigh.
A patient can develop shock without skin or mucosal signs. Always prioritize ABC assessment. A sudden drop in blood pressure with a thready pulse in the context of a known allergen exposure is a medical emergency requiring immediate intervention.
