학습 자료 둘러보기 →The priority nursing intervention before initiating a magnesium sulfate infusion for severe preeclampsia is to ensure the immediate availability of the antidote, calcium gluconate, at the bedside. This is a non-negotiable patient safety standard due to the narrow therapeutic index of magnesium sulfate and the rapid onset of life-threatening toxicity.
Magnesium sulfate toxicity can progress quickly from loss of deep tendon reflexes to respiratory depression (respirations <12/min) and cardiac arrest. Calcium gluconate (10% solution, typically 1 g IV push over 3-5 minutes) must be physically present and checked before the infusion pump is started.
Serial assessments during the infusion are vital. The earliest sign of magnesium toxicity is the loss of deep tendon reflexes (areflexia). The nurse should hold the infusion and notify the provider immediately if DTRs are absent, respirations are less than 12 per minute, or urine output falls below 30 mL/hour.
