Position the infant in a semi-Fowler's position by elevating the head of the bed 15 to 30 degrees. This uses gravity to lower the diaphragm and minimize abdominal pressure on the thoracic cavity, optimizing lung expansion.
Supportive care is the cornerstone of bronchiolitis management. Avoid interventions that lack evidence or increase distress. Bronchodilators and chest physiotherapy are not routinely recommended. Frequent oral feeding should be paused if the respiratory rate exceeds 60 breaths/min due to aspiration risk; consider nasogastric or IV fluids instead.
Continuously monitor oxygen saturation and respiratory status. Suction the nares gently only if secretions are obstructing the airway, as aggressive suctioning can worsen edema and cause trauma.
