학습 자료 둘러보기 →For a child with burns over 25% TBSA, signs of burn shock (tachycardia, tachypnea, narrow pulse pressure) indicate an immediate need for IV fluid resuscitation using lactated Ringer's solution. Restlessness is a hallmark of pediatric hypovolemia.
Calculate fluid needs using the Parkland formula: 4 mL x weight (kg) x % TBSA. Administer half of the total volume in the first 8 hours from the time of injury, not arrival.
Do not apply cool dressings to burns over 10% TBSA due to the risk of severe hypothermia. Delay pain medication until IV access is established and volume status is stabilized to prevent vasodilation and worsening hypotension.
