Assess the 5 Ps: Pain (especially with passive stretch), Paresthesia, Pallor, Pulselessness, and Poikilothermia. In sedated children, rely on objective signs: Doppler pulses and capillary refill time greater than 2 seconds.
Do not delay notification of the provider if signs of impaired perfusion appear. An escharotomy may be needed urgently. Administering pain medication before assessment can mask the critical early symptom of disproportionate pain.
