Upon recognition, the nurse must immediately obtain blood cultures (peripheral and from any central line) and administer the prescribed empiric broad-spectrum antibiotics without delay. The goal time-to-antibiotic (TTA) is within 60 minutes of presentation, as each hour of delay increases mortality risk.
Do not delay antibiotics for a complete workup. A lack of obvious infection source does not rule out sepsis, as the inflammatory response is blunted. A rectal temperature, enemas, and intramuscular injections are contraindicated due to the risk of mucosal breach and bleeding.
