학습 자료 둘러보기 →Suspect acute pyelonephritis when a patient presents with costovertebral angle (CVA) tenderness and high fever (>38.3°C) with chills. This reflects inflammation of the renal parenchyma and systemic involvement.
Lower UTI (cystitis) typically presents with localized symptoms: suprapubic tenderness, dysuria, urinary frequency, and urgency. Fever is usually absent or low-grade.
Do not rely on urinalysis alone to distinguish upper from lower UTI. Always assess for CVA tenderness and systemic signs (fever, chills, nausea) to rule out pyelonephritis, which requires urgent antibiotic therapy and possible hospitalization.
