학습 자료 둘러보기 →When a patient with pneumonia on high-flow nasal cannula (HFNC) has a dropping SpO2 despite FiO2 60%, suspect a physiology similar to ARDS with significant intrapulmonary shunting due to dorsal atelectasis.
Immediately position the patient in a prone position. This uses gravity to recruit collapsed dorsal alveoli, making ventilation more homogeneous and dramatically improving V/Q matching.
Prone positioning is a high-priority nursing intervention. Do not delay by first maximizing HFNC settings or placing the patient supine, which worsens the underlying atelectasis. Ensure the airway and lines are secure during the turn.
