학습 자료 둘러보기 →The highest priority after total knee replacement surgery is monitoring for pulmonary embolism (PE). A sudden triad of severe chest pain, shortness of breath, and anxiety is a classic presentation and constitutes a medical emergency.
This occurs due to Virchow's triad: venous stasis from immobility, endothelial injury from surgery, and a postoperative hypercoagulable state. A thrombus, often a deep vein thrombosis (DVT), embolizes to the lungs, causing a ventilation-perfusion mismatch and acute hypoxemia.
Do not dismiss this as anxiety or incisional pain. Immediately assess vital signs, apply oxygen, notify the Rapid Response Team, and prepare for diagnostic tests like a CT pulmonary angiogram. Expected findings like 50 mL of serosanguineous drainage or localized surgical site warmth are not the priority.
