This occurs as the iliopsoas muscle pulls the proximal fragment into flexion and external rotation, while the distal fragment is rotated by the gluteal muscles.
Always compare the position of the patella and the medial malleolus to the unaffected side. A leg that is externally rotated and 1-2 cm shorter is highly suggestive of a displaced femoral neck or intertrochanteric fracture. This finding helps differentiate a fracture from a hip contusion or pelvic injury.
Do not attempt to straighten or rotate the leg for a better view. Movement of the fracture fragments can cause further soft tissue damage, increase bleeding, or dislodge a clot. Immobilize the limb in the position found and use a scoop stretcher for transfers.
