학습 자료 둘러보기 →Suspect leukostasis in any patient with acute leukemia and a WBC count exceeding 100,000/mm³ who presents with acute respiratory distress or neurological changes. The clinical diagnosis is based on symptoms, not imaging.
The nurse's priority action is immediate physician notification and preparation for definitive therapy, which includes leukapheresis and/or cytoreductive chemotherapy with agents like hydroxyurea.
While awaiting definitive treatment, provide supportive care: administer high-flow oxygen to target SpO2 > 92%, maintain the patient on bed rest to minimize oxygen consumption, and ensure adequate hydration unless contraindicated.
Avoid red blood cell transfusions unless absolutely necessary, as they can increase blood viscosity and worsen the crisis. If transfusion is critical, it should be done slowly and after leukapheresis has been initiated.
