The foundational step is to establish large-bore IV access (14-18 gauge) in the antecubital fossa or other large vein. This allows for rapid infusion of crystalloids and blood products to restore intravascular volume.
Do not delay fluid resuscitation for diagnostic procedures like NG tube placement or endoscopy. Definitive airway management in a severely volume-depleted patient carries a high risk of cardiovascular collapse if circulation is not restored first.
