When the kidney itself is injured, urea nitrogen and creatinine rise together. A disproportionate rise in urea nitrogen alone reflects reduced renal perfusion with increased urea reabsorption, that is, dehydration or another prerenal cause. A high ratio is the signature of a prerenal state.
