When the peripheral hormone is deficient, the pituitary secretes more stimulating hormone. An elevated thyroid-stimulating hormone with a low free T4 localizes the problem to the thyroid gland itself, that is, primary hypothyroidism. In secondary hypothyroidism the stimulating hormone would also be low.
