The erythrocyte sedimentation rate depends on rouleaux formation. Cells with an abnormal shape, such as sickle cells, cannot form rouleaux, so sedimentation is slow and the result is falsely low. Increased fibrinogen, anemia, and pregnancy all raise the rate.
