The priority intervention is nasogastric (NG) tube insertion for decompression. This directly removes fluid and air, lowering intragastric pressure and mitigating the risk of vomiting and subsequent aspiration pneumonia.
Do not administer oral fluids or medications to a client with absent bowel sounds and severe distension. Ambulation and antiemetics are secondary measures that do not address the immediate, life-threatening risk of aspiration.
