학습 자료 둘러보기 →For a patient with sudden chest pain, tachypnea, hypotension, and hypoxemia (SpO2 88%) after a total hip replacement, suspect a pulmonary embolism (PE). The immediate priority is to apply supplemental oxygen to target a saturation above 90%.
Simultaneously, notify the physician or rapid response team. Definitive treatment requires emergent diagnosis via CT pulmonary angiography and may include anticoagulation or thrombolysis for hemodynamically unstable patients.
Do not delay oxygen therapy for positioning or pain medication. Avoid maneuvers that increase venous return, such as placing the patient in Trendelenburg, which can worsen right ventricular strain.
