학습 자료 둘러보기 →The tripod position is a critical compensatory mechanism in epiglottitis. The child sits upright, leans forward, and extends the neck to use gravity and neck muscles to splint the swollen epiglottis open. A child who refuses to lie down and fights to maintain this posture is signaling a critically narrowed airway and impending fatigue.
When the child can no longer maintain the tripod position due to exhaustion, the airway will completely obstruct. This is a pre-arrest sign requiring immediate notification of a skilled airway provider (anesthesiologist or ENT) and preparation for emergency intubation or surgical airway.
Do not force the child to lie down, examine the throat with a tongue depressor, or cause agitation. These actions can trigger laryngospasm and complete airway loss. Keep the child calm in a position of comfort, often on the parent's lap, and provide blow-by oxygen if tolerated.
