Alkali agents like calcium hydroxide cause saponification, which disrupts cell membranes and allows rapid, deep penetration. The severity is graded by limbal ischemia and corneal clarity, not pain level.
Do not rely on pain scores to gauge burn severity. Severe alkali burns can be initially painless due to nerve destruction. Immediate, copious irrigation with normal saline or lactated Ringer's for at least 30 minutes is the priority before any detailed exam.
