An IOP of 45 mmHg is a critical emergency (normal: 10-21 mmHg). Irreversible optic nerve damage and permanent vision loss can occur within hours if the pressure is not relieved immediately.
Do not delay miotic administration for analgesia or surgical preparation. Medical IOP reduction is the first step to break the acute attack and reduce surgical risks. Always verify the provider's order before administration.
