Assess the stoma every 4 hours for color, size, and mucus production. A dusky, cyanotic, or black stoma indicates ischemia or necrosis and requires immediate reporting.
Do not confuse normal postoperative edema with signs of venous congestion. A sudden increase in size with a tense, shiny surface is an emergency.
