학습 자료 둘러보기 →The priority assessment in bladder cancer is frank hematuria with large clots, which indicates brisk, active bleeding. This can rapidly lead to hemorrhagic shock (tachycardia, hypotension) and clot retention obstructing urine outflow.
Immediate interventions include establishing large-bore IV access, initiating fluid resuscitation, and preparing for continuous bladder irrigation (CBI) with a three-way catheter. Monitor hemoglobin and coagulation studies closely.
Never ignore a sudden increase in clot size or onset of frank blood. A rigid, distended bladder with severe pain suggests clot retention and requires urgent catheterization to prevent bladder perforation.
