학습 자료 둘러보기 →The immediate priority for a client with hydronephrosis from a ureteral obstruction is to monitor urine output and kidney function. The back-pressure from the blockage causes progressive, often silent, renal parenchymal damage that can become irreversible.
Accurate intake and output measurement is critical. Following relief of the obstruction, the nurse must vigilantly assess for post-obstructive diuresis, defined as a urine output exceeding 200 mL/hour for two consecutive hours, which can lead to severe fluid and electrolyte imbalances.
Do not encourage increased fluid intake while the obstruction is still present. Forcing fluids will exacerbate the hydronephrosis and increase intrarenal pressure, accelerating kidney damage.
