Assess for costovertebral angle (CVA) tenderness by gently percussing the back over the kidney. Pain may be intermittent and is often the primary symptom, even without urinary complaints.
Do not confuse with pyelonephritis, which typically presents with fever, chills, and dysuria. Decreased urine output is a late sign of severe bilateral obstruction and requires immediate intervention.
