학습 자료 둘러보기 →In ureteral obstruction, the immediate threat is not dehydration but rising intrarenal pressure. The priority is to assess for postrenal acute kidney injury (AKI) and urosepsis. Monitor urine output strictly; a drop below 30 mL/hr signals oliguria and declining function.
Assess for signs of infection such as fever, chills, and costovertebral angle tenderness. Urine stasis behind a blockage is a breeding ground for bacteria, making urosepsis a life-threatening complication. Obtain a urine culture if infection is suspected.
Do not force fluids or administer diuretics. These interventions increase urine production against a fixed blockage, dangerously elevating hydrostatic pressure and accelerating renal parenchymal damage.
