The immediate priority intervention is to notify the provider and prepare for emergent hemodialysis. Dialysis is the definitive treatment to remove the uremic toxins causing the inflammation. Monitor closely for signs of tamponade: Beck's triad (hypotension, muffled heart sounds, jugular venous distension) and pulsus paradoxus.
Do not delay treatment for lab values like a BUN of 150 mg/dL or creatinine of 8.5 mg/dL. While critically elevated, they indicate a chronic state. The friction rub signals an acute, active inflammatory process that can decompensate within hours.
