Immediately institute strict bed rest and elevate the head of bed to 15-30 degrees. This position promotes cerebral venous outflow to lower ICP without critically compromising Cerebral Perfusion Pressure (CPP).
Minimize all environmental stimulation: dim the lights, reduce noise, limit visitors, and cluster care. Any sudden spike in blood pressure or ICP can precipitate catastrophic rebleeding.
Never place the patient in Trendelenburg position; it increases ICP. Avoid activities that raise intrathoracic pressure, such as deep breathing exercises or Valsalva maneuver. Pain management is secondary and must not mask neurological deterioration.
