This position optimizes cerebral venous drainage via the jugular veins, reducing intracranial blood volume without compromising arterial inflow.
The Monro-Kellie doctrine dictates that the skull is a fixed compartment. When cerebral edema increases brain volume, venous outflow must be maximized to compensate. A flat position or a turned head can kink the jugular veins, causing venous pooling and a rapid rise in ICP.
Avoid any intervention that triggers the Valsalva maneuver, such as encouraging coughing. This increases intrathoracic pressure, which is directly transmitted to the cranium and can cause a dangerous ICP spike. Maintain a calm environment and cluster care to minimize sustained noxious stimuli.
