학습 자료 둘러보기 →The highest priority action for suspected elevated intracranial pressure (ICP) is to elevate the head of the bed to 30 degrees. This simple maneuver uses gravity to promote cerebral venous outflow, directly reducing intracranial blood volume and ICP.
Maintain the head and neck in a midline, neutral position to prevent kinking of the jugular veins, which would impede venous drainage. Avoid hip flexion, as this can increase intra-abdominal pressure and impede venous return.
This intervention is contraindicated if the patient is hemodynamically unstable. Always ensure the cerebral perfusion pressure (CPP) is maintained above 60 mmHg when adjusting the head of bed. Never instruct a patient with elevated ICP to perform the Valsalva maneuver or cough forcefully, as these actions can cause a dangerous spike in pressure.
