It suggests peritoneal carcinomatosis, where malignant cells obstruct lymphatic drainage, shifting the diagnosis toward advanced malignancy.
While a pelvic mass with ascites can be benign (Meigs' syndrome), this is rare. In practice, new-onset ascites in a woman over 50 with an adnexal mass is a red flag. Diagnostic workup includes a CA-125 tumor marker and transvaginal ultrasound, but definitive diagnosis requires surgical biopsy.
Do not dismiss non-specific GI symptoms as benign in this population. Absence of irregular bleeding does not rule out ovarian cancer. Always assess for shifting dullness or a fluid wave to detect ascites early.
