This severe presentation is a sympathetic storm, an uncontrolled autonomic crisis. The body's regulatory systems fail, leading to a loss of central nervous system homeostasis. This can be triggered by withdrawal from potent adulterants like medetomidine, an α2-adrenergic agonist found in illicit opioids.
Immediate intervention is required. Uncontrolled sympathetic outflow can rapidly lead to rhabdomyolysis, severe dehydration, and cardiovascular collapse. Monitor core temperature, provide aggressive cooling, and manage agitation with sedation while ensuring airway protection.
