학습 자료 둘러보기 →In cardiogenic shock, the primary problem is pump failure, not volume loss. The priority is to improve myocardial contractility and cardiac output with inotropic agents (e.g., dobutamine) as ordered.
Assess for signs of end-organ hypoperfusion: urine output <30 mL/hr, altered mentation, and cool, clammy skin. Early recognition and intervention are critical.
Avoid routine fluid boluses in cardiogenic shock unless specifically indicated for right ventricular infarction. Volume overload can precipitate pulmonary edema. Trendelenburg position is contraindicated as it increases preload and intrathoracic pressure.
