학습 자료 둘러보기 →Suspect pulmonary tuberculosis in a child presenting with an indolent symptom triad: low-grade fever, night sweats, and a persistent dry cough lasting 3 weeks or longer. This contrasts sharply with the acute, high-fever onset of bacterial pneumonia.
The pathophysiology involves a slow, cell-mediated immune response leading to granuloma formation. Systemic symptoms result from chronic cytokine release. A non-productive cough is common in early pediatric disease because cavitary lesions are less frequent than in adults.
Do not rule out TB based on a negative tuberculin skin test or IGRA in very young or immunocompromised children, as anergy can occur. Collect early morning gastric aspirates for culture, as children often swallow sputum.
