학습 자료 둘러보기 →For a patient with community-acquired pneumonia unresponsive to 2 weeks of antibiotics, the immediate priority is to initiate airborne precautions. This requires placing the patient in a negative pressure isolation room and using an N95 respirator for all entry.
The primary clinical concern is ruling out pulmonary tuberculosis (TB). A persistent respiratory infection despite broad-spectrum treatment fits the clinical picture of TB, an airborne disease requiring the highest level of respiratory protection to prevent healthcare-associated transmission.
Do not downgrade to droplet precautions (surgical mask) or standard precautions only. A surgical mask on the patient during transport is a droplet measure and insufficient for suspected airborne pathogens. Empiric airborne isolation must be maintained until TB is ruled out by negative sputum smears or nucleic acid amplification tests.
