학습 자료 둘러보기 →For a child with severe head injury, the priority intervention is maintaining the head of bed at 30 degrees with the head in neutral midline alignment. This position promotes cerebral venous outflow via gravity, directly reducing intracranial blood volume and lowering ICP.
Avoid neck flexion, rotation, or hyperextension, which can compress the jugular veins and cause a rapid, dangerous spike in ICP. This is a continuous management strategy, not a one-time action.
Do not perform routine scheduled suctioning. Suctioning is a noxious stimulus that triggers coughing and the Valsalva maneuver, causing a significant transient spike in ICP. Suction only when clinically indicated and pre-oxygenate.
