학습 자료 둘러보기 →The immediate priority for a patient with Cushing's triad (bradycardia, widened pulse pressure, irregular respirations) and an ICP of 25 mmHg is to elevate the head of the bed to 30 degrees. This non-invasive action promotes cerebral venous outflow and CSF drainage via gravity, directly lowering ICP to prevent cerebral herniation.
Ensure the head is maintained in a midline, neutral position to prevent jugular vein compression or kinking, which can further impede venous return and raise ICP. Avoid hip flexion greater than 90 degrees during positioning.
Do not administer hypotonic IV fluids or increase fluid rates, as this can worsen cerebral edema. Maintain normothermia and avoid fever, as hyperthermia increases cerebral metabolic demand and ICP. Suctioning and painful stimuli should be minimized and preceded by preoxygenation to avoid transient ICP spikes.
