Use the Confusion Assessment Method (CAM) for systematic screening. Key features include acute change from baseline, inattention, disorganized thinking, and an altered level of consciousness that can be hypoactive or hyperactive.
Do not mistake hypoactive delirium (quiet, withdrawn) for depression or fatigue. It carries a higher mortality risk than hyperactive delirium and is often missed. Any acute change in mental status warrants immediate medical evaluation.
