학습 자료 둘러보기 →In an elderly patient with a fall preceded by dizziness, orthostatic vital signs are the highest priority assessment. A finding of systolic BP drop ≥20 mmHg or diastolic BP drop ≥10 mmHg with a heart rate increase to 110 bpm signals hemodynamic instability and possible cardiac syncope, not simple dehydration.
Cardiac syncope is an underestimated cause of unexplained falls. The compensatory tachycardia indicates the body is struggling to maintain cerebral perfusion. This presentation is a red flag for life-threatening arrhythmias or structural heart disease and requires immediate intervention, such as continuous cardiac monitoring and urgent provider notification.
Do not dismiss orthostatic hypotension as simple dehydration in a post-fall elderly patient. The combination of a significant BP drop and marked tachycardia is a critical finding that distinguishes a potential cardiac event from a less acute fluid volume deficit.
