Perform the HINTS exam (Head Impulse, Nystagmus, Test of Skew) in patients with continuous vertigo. A positive head impulse test (corrective saccade) strongly suggests a peripheral cause like labyrinthitis.
Labyrinthitis is a diagnosis of exclusion. Central causes like cerebellar stroke can mimic it. The presence of vertical nystagmus, skew deviation, or inability to ambulate are red flags for a central lesion requiring immediate neuroimaging.
