학습 자료 둘러보기 →A corneal foreign body presents with acute, sharp, stabbing pain that worsens with blinking due to the eyelid scraping the particle across the densely innervated corneal epithelium. The pain is immediate and severe, driven by nociceptor activation from the ophthalmic branch of the trigeminal nerve.
Suspect a corneal foreign body when a patient with a high-risk occupation, like a mechanic, reports a sudden onset of intense pain and a foreign body sensation. Visual acuity may be affected if the particle is in the visual axis, but the hallmark is the pain disproportionate to visible findings.
Do not delay a slit-lamp examination with fluorescein staining. A seemingly minor symptom of mild irritation does not rule out a penetrating injury. Always evert the upper eyelid to rule out a subtarsal foreign body, which can cause linear corneal abrasions with each blink.
